 The St. Lawrence CYO Basketball  program is run by volunteers.  All Parents/Guardians are asked to please participate in the program in some way.  You must select at least one activity listed below +

Medical Release

I hereby give my permission for any and all medical attention necessary to be administered to my child(ren) registered with this submission  in the event of an accident, injury, sickness, etc. under the direction of the Coaches and/or Assistant Coaches of the team.  I assume responsibility for payment for any such treatment.  If I can not be reached in an emergency, my child(ren)'s coach and or assistant coach is hereby authorized to act on my behalf.

I hereby consent to my child(ren)'s participation in the St. Lawrence CYO Basketball Program and I hereby agree to indemnify and hold free and harmless the Coaches, Assistant Coaches, other team members, officials and others affiliated wiht the program from any and all liability arising out of my child's participatin in the program, whether liability is caused by, or arises out of, the negligence of the program, coaches, other team members, officials, others affiliated with the program or otherwise.
